Skorobogatov said officials are now taking steps, including putting all assembly staff through competence tests and additional training, to make sure such malfunctions don't happen again.

But the failure is worrisome, since Soyuz is the only human-rated spacecraft used to get people to and from the space station.

Roscosmos is due to hold a press conference Thursday to further detail the findings of the accident probe.

The current ISS crew, the ESA's Alexander Gerst, NASA's Serena Aunon-Chancellor, and Roscosmos' Sergey Prokopyev, are expected to return to Earth around December 20, a week after their originally-scheduled December 13 descent.

What happened during the flight?


Russian Federation suspended all launches after the accident on October 11, unprecedented for Russia's post-Soviet manned launches, that saw the rocket fail minutes after blast-off.

The incident occurred on 11th October as has been traced to a malfunction that occurred in the detector that signals separation of the rocket's first and second stages. The space capsule carrying the two men ripped away from the damaged rocket, then plunged back to Earth.

After investigating the incident, Russia's space agency, Roscosmos, determined that one of the rocket's boosters failed and stuck to the main rocket body instead of peeling off.

Igor Skorobogatov, who headed the inquiry, said on Thursday that the issue was linked to the "deformation" of a sensor part.

"We have conclusively established that the only place where [damage to the sensor] could have taken place was the Baikonur cosmodrome during the rocket's assembly". It's relied upon by NASA, Europe, Russia, and other partners.

"The abnormal separation was caused by the nonopening of the lid of the nozzle meant to separate aside Block D oxidizer tank, due to the deformation of the separation sensor pin [which was bent by 6 degrees and 45 minutes]", Roscosmos officials said in a statement today.

Since then, Nasa has paid Russian Federation for seats on its Soyuz rockets to ferry its astronauts to the station.
